You immediately reflect a piece of jewelry on the spot
You receive a Reflecting Mist item as a drop
The first scenario is common; the second is extremely uncommon. You may encounter dozens of Mists before ever seeing the actual currency drop.
If you're trying to maximize Mist appearances:
Chain fast T16 maps with strong layouts (Strand, Dunes, Cemetery, Crimson Temple)
